The Husband of Pamela Ashley demands accountability
An inquest into the death of a Perth woman who died at Armadale Hospital commences in the Coroner’s Court of Western Australia tomorrow.

Pamela Ashley a 64 year old woman from Champion Hills died in Armadale Hospital on 3rd February 2016, after being transferred from the hospital’s Emergency Department to its older adults mental health service, Banskia Ward.

Pamela’s husband is calling for the truth to come out about his wife’s death and for changes in WA’s hospital and mental health care systems as a consequence.

“Pamela presented to Armadale hospital with a mental disturbance which was not life threatening. Pamela’s husband is hoping that the inquest will determine whether there was a systemic failure in Pamela’s care or whether any action or inaction of the hospital contributed to Pamela’s death and whether observation and prompt intervention could have saved her life.”
