Bakcgrounder …

A roadmap to better health care….

Tasmanian HealthPathways is a result of a successful collaboration between Tasmania Medicare Local, Tasmanian Health Organisations and the Department Health and Human Services aimed at connecting care across Tasmania.

Development of Tasmania HealthPathways’ web portal helps establish an agreed ‘care map’ for GPs and other clinicians to assist in more consistent and streamlined diagnoses, referrals and treatment strategies.

Health professionals across the state will initially have access to more than 80 localised Tasmanian pathways in diabetes, cardiology and immunisation. They will also have access to more than 400 unlocalised pathways from NZ that may also be useful to clinical practice.

Pathways in respiratory disease and neurodegenerative disease will be available later in the year with many more currently in development. By July 2016 there will be at least 130 localised Tasmanian pathways available. Over time, all pathways on the web portal will be localised to Tasmania.

This TML project is a game-changer in terms of better connecting primary and acute health care for more Tasmanians.
